Obtaining a Sacramental Record

In order to obatain information from our sacramental records please provide the following information. You can expect a record mailed to you within two weeks. In the details section please include birth year, date, parents names, etc.

A $20 donation to the church per records request is kindly requested. All donations can be mailed directly to the church at 263 Mulberry St. New York, NY 10012.
